Who do we serve? (Eligibility)

To qualify for PACE, you must:

1. Be 55 Years of age or older

2. Be certified by Vermont to need a nursing home level of care (the Intake Coordinator can help with this step)

3. Live in the PACE service area: Chittenden, southern Grand Isle, Rutland, and northern Bennington Counties. [ See Map ]

4. Desire to live at home and be able do so safely, as determined by the PACE Health Team
