How does it all Work?

PACE is a health care program that supports older adults in their homes and communities.  PACE combines all medical and social services into one program.  Services are provided by a single Health Team that works closely with the PACE participant. 

The Health Team includes a doctor, nurse, physical therapist, occupational therapist, social worker, nutritionist, activities director, and caregivers. [more]

The PACE Centers offer many health services in one location: doctor's office, therapy gym, adult day program, social work and nutrition offices. [more]

Medical Care - PACE's Doctor, Nurse Practitioner, and Nurses specialize in caring for older adults. Participants are seen as needed; often the same day appointments are requested. [more]

Therapies - Physical Therapy, Occupational Therapy, Nutrition Services, and Social Work are all available at the PACE Centers. [more]

Adult Day Programming - Meet new people; rediscover lost hobbies; enjoy a meal. Adult Day programming is more than Bingo! Field trips, musical performances, gardening and book clubs are regular offerings. [more]

Medical Transportation - Door-to-door service in wheelchair-accessible vans gets participants to and from PACE Centers and other medical appointments. [more]

In-Home Care - Licensed Nursing Assistants (LNAs) and Personal Care Attendants (PCAs) visit participant homes to provide assistance with tasks such as personal hygiene and bathing, getting dressed, transferring, preparing meals, grocery shopping, and light housekeeping. [more]

Support for Family - Pace is on call 24-hours/day, 365 days/year. Respite allows family to take vacation or attend to other responsibilities. Support from the PACE Social Worker is available to family members. [more]

End of Life Care - PACE supports elders wherever their care needs take them. PACE provides additional in-home services and supports at the end of life. PACE can also arrange for nursing home or hospital care. [more]

How does it all work? The PACE Care Plan - Care Plans are written for each participant, based on each person's unique needs and personal goals. [more]

PACE participants must receive all needed healthcare, including primary care and special physician services from PACE Vermont or from an entity authorized by PACE Vermont. Participants may be fully and personally liable for the cost of unauthorized or out-of-Network Services.  Please see our Provider Network for a complete list of providers currently available through PACE.
