The PACE Center

The PACE Centers offer many health services in one location: doctor’s office, therapy gym, adult day program, social work and nutrition offices.

Participants can visit the PACE Center to receive routine medical care, social services, therapies, have lunch, and meet with members of the Health Team. All PACE Health Team members are based at the PACE Center, and can be easily reached by calling the main PACE phone number, or by coming in for a visit.

PACE Centers are open Monday through Friday, 8:00 a.m. to 5:00 p.m. Time at the Center can range from a monthly visit, to part of a day, to 5 full days per week.

Burlington Area: Fanny Allen Campus 786 College Parkway Colchester, VT 05446 802.655.6700 [GOOGLE MAP]	When entering the Fanny Allen Campus, make an immediate right hand turn. The drive wraps around the PACE Center to our parking area and main entrance.

Rutland Area: Maples Senior Living Community 3 General Wing Road Rutland, VT 05701 802.776.2900 [GOOGLE MAP]	General Wing Road is off of Stratton Road, one block from the hospital. Follow General Wing Road to the end and turn right. You will see the PACE Center entrance ahead and to your right.
