PACE Vermont covers and organizes all services that are approved by the Health Team as medically necessary.  At a minimum, this includes any services that are covered by Medicare or Medicaid.

The PACE Vermont Centers have a primary care office, nursing clinic, physical and occupational therapy gym, social work office, and space for nutritional counseling.  In addition, the PACE Centers offer social programs, serve lunch, and provide personal care such as showers. 

Services provided in the home include personal care, chore services, and skilled services.  These services can be provided during the day, evenings, and on weekends.  Members of the PACE Health Team are also able to make home visits.

Services provided in the community include: pharmacy (medications can be delivered home or picked up at the Center); procedures in the hospital; Emergency Room; inpatient rehabilitation; visits to specialists including dentistry and optometry. 

Finally, PACE provides transportation to and from all medical appointments, including the PACE Centers.

PACE participants must receive all needed healthcare, including primary care and special physician services from the PACE Vermont or from an entity authorized by PACE Vermont.  Participants may be fully and personally liable for the cost of unauthorized or out – of – PACE Program Agreement Services.
