The goal of PACE is to help its participants stay as independent as possible, for as long as possible, in the housing of their choice.
 
PACE participants receive all the care they need through one team, including:
• All services normally covered by Medicare and Medicaid
• Transportation to medical appointments
• Medications automatically refilled and delivered home
• In-Home Help including chore services and personal assistance
• Ongoing Preventive Care including physical and occupational therapies
• Recreation at the PACE Center
• Respite for caregivers
 
PACE services are organized and provided by the PACE Health Team. This team includes the following members:
• Doctors, Nurse Practitioners, and Nurses
• Physical and Occupational Therapists
• Social Workers and Case Managers
• Nutritionists
• Activities Staff
• Direct Care Workers – Licensed Nurse’s Aids (LNAs) and Personal Care Attendants (PCAs)
• Coordinators and Schedulers for in-home care and transportation
 

All members of the Health Team provide services at the PACE Center . Services are also provided in the home.

The PACE Center has a doctor’s office, therapy gym, and adult day program. Our Centers are open Monday through Friday, 8:00 a.m. until 5:00 p.m. The time participants spend at the Center depends upon the services they want and need, and can range from no time at the Center, to part of a day, to 5 full days per week.

Health Team members are able to make home visits when needed. Caregivers can help with daily tasks like light housekeeping and personal care during the daytime, evenings, and on weekends.

The PACE Health Team coordinates appointments and transportation to specialists including the dentist and optometrist, in-patient and out-patient hospital services, and rehabilitation in nursing homes.

All services approved by PACE are 100% covered with no cost to the participant. PACE eliminates co-payments, deductibles, and doughnut holes. There are no hidden fees, and no claims forms. Some participants pay a monthly fee to participate in the program.

 
PACE participants must receive all needed healthcare, including primary care and special physician services from the PACE Vermont or from an entity authorized by PACE Vermont. Participants may be fully and personally liable for the cost of unauthorized or out – of – PACE Program Agreement Services.
