Medical Care

    

“I love PACE’s team concept, where the physician is one of several professionals, and everyone is an equal partner at the table.  That’s what an older person, and their family, needs.”  - Susan, PACE participant’s daughter.

PACE’s Doctor, Nurse Practitioner, and Nurses specialize in caring for older adults.  Participants are seen as needed; often the same day appointments are requested. 

PACE helps manage complex medical conditions.  The PACE Team works closely with medical specialists to ensure participants remain as healthy as possible. 

PACE automatically refills and delivers medications home. 

This service is one benefit of the PACE program. PACE participants receive all their health care from the PACE Health Team and Provider Network.
