SOCIALWORKER (MSW) - Rutland Site

Job Summary:
The clinical social worker promotes and helps maintain the mental and social health of participants through assessment, treatment, teaching and counseling.  Facilitates communication between participants, their family and the Interdisciplinary Team.   The clinical social worker is knowledgeable regarding social systems and institutions and individual behavior and can apply appropriate interventions to meet the needs of the participant and family

Qualifications:
Requires Masters in Social work.   Working knowledge of the interdisciplinary model of care, having effective communication skills. A minimum of one years’ experience working with frail elders and with persons with cognitive impairment is required.  Must be able to relate well with elders and their families to deal with sensitive issues and facilitate problem solving.  Familiar with community resources.  Able to work effectively in a team environment.  Must have a valid driver’s license and reliable transportation.

Essential Functions:

• Promotes and practices the PACE Vermont Mission and Values.
• Completes initial and periodic assessments; Initial and on-going assessments are completed prior to the scheduled IDT meeting and the development of the Plan of Care (POC) is completed during the IDT meeting and finalized with the signature of the participant/caregiver, within one week of the IDT meeting.  Signature by the participant/caregiver indicates agreement with the POC.
• Actively participates in interdisciplinary evaluation, assessment, and plan of care for all participants.
• Confers with participant and family to identify participant goals and expectations
• Communicates participant changes to team members.
• Coordinates the plan of care for 24 hour delivery
• Documents the assessment and plan of care development, including the timeframe, in the medical record. .
• Responds to identified needs of participants and their families in a clinically sound, therapeutic manner on an ongoing basis
• Provides counseling and crisis intervention, and addresses mental health needs as they arise.
• Maintains current information related to community resources, and federal and state guidelines.
• Facilitates participant and family adjustment to lifestyle changes.
• Collaborates with and respects PACE Vermont team members, referral sources and outside agencies.
• Assists participants in obtaining housing; may assist with  the development of money management systems, including assisting participants and family with the Long-Term Care Medicaid application; assures current Medicaid eligibility
• Conducts family support groups, education or training sessions and routine family caregiver meetings for education, support and discussion.
• Coordinates with mental health related providers including drug and alcohol treatment to arrange appointments and share pertinent information with participant’s consent.
• Clinical chart documentation is completed according to established documentation standards and in a timely manner.
• Conducts monthly chart audit of Activity Coordinator per chart audit tool
• Maintains required statistical data.


PHYSICAL THERAPIST, Colchester Site

Job Summary:

Evaluates and reassesses patients to assess mobility level and other functional abilities.  Provides and recommends therapeutic exercise, gait training, ambulation, etc.  Plans and develops group exercise programs for maintenance therapy for participants.  Responsible for the overall physical therapy program at PACE.

 

Qualifications:

Licensed to practice Physical Therapy in Vermont .  Possess a valid driver’s license and reliable transportation.  One year experience working with the frail/elderly.  Able to demonstrate clinical competency with frail/elderly population.  Will supervise program aides and physical therapy assistants as needed.  CPR certification required

 

Essential Functions:

  • Develops treatment program when indicated, sets goals and provide treatment.
  • Reviews and completes patient assessments and discharge summaries.  Plans and develops maintenance, exercise and therapy programs for participants.
  • Provides training to family members, friends, and PACE staff members for those parts of the treatment plan that they can carry out.
  • Provides supervision to program aides and other staff who are implementing a physical therapy treatment program.
  • Communicates participant changes to team members
  • Participates in the development of the IDT Plan of Care
  • Participates in the coordination of a 24 hour care service.
  • Completes all documentation in the medical record as required
  • Conducts monthly chart audit of Occupational Therapy role per chart audit too

Supervision:  Administrative and clinical supervision for this position is provided by the PACE Vermont Center Manager.

Physical Demands:

Ability to move about freely, lift (50lbs) and turn participants, have the manual dexterity and visual acuity to see and work with participants, be able to hear and orally communicate.

OCCUPATIONAL THERAPIST - Rutland Site

Job Type: Contracted or Per Diem, 0.5 - 0.6 FTE

Job Summary:

The Occupational Therapist will evaluate and reassess participants upon referral from the inter-disciplinary team.  Will provide and recommend therapeutic exercises and assist in the adaptation to the participant’s environment. 

 

Qualifications:

Graduate of an OT curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American OT Association. At least two years experience as an OT, minimum one year experience working with a frail/elderly population.  Must be licensed as an OT in Vermont , valid driver’s license, and reliable transportation.  Demonstrates clinical competency with frail elder population.  Must have successfully completed a CPR program.

 

Essential Functions:

  • Promotes and practices the PACE Vermont Mission and Values
  • Conducts initial assessments at Intake and reassessments annually.  Initial and on-going assessments are completed prior to the scheduled IDT meeting and the development of the Plan of Care (POC) is completed during the IDT meeting and finalized with the signature of the participant/caregiver, within one week of the IDT meeting.  Signature by the participant/caregiver indicates agreement with the POC.
  • Develops treatment program when indicated, sets goals and provides treatment.
  • Provides individual therapy for participants as needed or prescribed in the care plan to increase or maintain strength, range of motion, endurance or dexterity in the upper extremities.
  • Assesses and provides treatment to improve perceptual-motor deficits, facial weakness, sensory deficits and cognitive limitations.
  • Provides treatment to increase self-care skills including bed mobility, feeding, hygiene, grooming, dressing, transfer ability, communication and homemaking as needed.
  • Carries out accepted principles and practices of occupational therapy for prevention of deformities and functional decline, and support improved function whenever possible.
  • Constructs or arranges for orthotic and adaptive equipment as needed.
  • Coordinates with home modification program for installation of equipment and other changes to support independence and safety.
  • Completes documentation of clinical services, records and signs treatments in participant’s medical record, signs and dates “Progress Notes.”
  • Provides training to family members, friends, and PACE staff members for those parts of the treatment plan that can be carried out by them.
  • Provides supervision to program aides and other staff who are implementing an OT treatment program.
  • Reviews assessments and discharges participants when appropriate.
  • Actively functions as a member of the interdisciplinary team, participates in case conferences when necessary.
  • Communicates participant changes to team members
  • Participates in the development of the IDT Plan of Care
  • Participates in the coordination of a 24 hour care service

HUMAN RESOURCES MANAGER

Primary Relationships:  Executive Director, Administrative & Fiscal Services Manager, co-workers, and public.

 

Objective:  The Human Resources Manager guides and manages the overall provision of Human Resources services, policies, and programs for the entire company.  The major areas directed are: recruiting and staffing; organizational and space planning; performance management; organization development; employment and compliance to regulatory concerns; employee orientation, development, and training; policy development and documentation; employee relations; company employee communication; compensation and benefits administration; employee safety, welfare, wellness and health; and employee services and counseling.

Essential Functions:

  • Oversee the recruitment and termination of employees of the PACE program.
  • Ensures compliance with all current regulations, laws, polices and procedures.  Sets up and maintains employee personnel and medical files, tracking all paperwork required by state and federal regulation and PACE Vermont, Inc. policies to ensure compliance
  • Acts as an internal consultant to supervisors and managers regarding the handling of employee relation’s issues.
  • Ensures compliance with Federal and State Employment Wage and Hours Laws.  Ensures the required federal and state mandatory posters are posted in the Center.
  • Ensures confidentiality and security of all employee files.
  • Reviews and forwards appropriate employment applications/resumes to hiring managers.  Updates and maintains internal job opportunities posting.
  • Recommends pay grades and participates in the budgeting process.
  • Oversees the performance appraisal system.  Generates and sends out monthly employee evaluation date lists and other employee reports.
  • Develops and monitors job descriptions for all PACE positions.  Develops and implements a grading system.
  • Promotes an effective work place for employees.  Develops a monthly staff newsletter.  Oversees an annual staff event.
  • Oversees the employee fringe benefit plan (i.e. medical, dental, life, retirement, and time off policy).  Acts as liaison with regard to insurance negotiation matters.
  • In conjunction with the Compliance Officer oversees Risk Management for the PACE organization.
  • Prepares work-related accidents and injuries reports with the worker’s compensations carrier.
  • Conducts new hire orientation including completion of required paperwork and review of employee manual policies and procedures.  Maintains new employee packets.  Inputs new employee information and benefits information; inputs address/telephone number changes.
  • Completes initial and annual verifications of employees’ professional licenses by checking on the State of Vermont Professional licensing website.
  • Provides benefits orientation for new employees and acts as a liaison with the insurance carrier to process additions, deletions, COBRA notification, status changes etc.
  • Responds to general requests/inquiries by employees regarding personnel policies, procedures, compensation and benefit
  • Provides training/education to staff in subject matters such as team building and communication skills. 
  • Follows all PACE Vermont, Inc. Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about employees, participants, clients and/or families.
  • Maintains safe working environment.  Follows PACE Vermont, Inc. Safety Policies and Procedures.
  • Participates in continuing education classes and any required staff and training meetings.  
  • Maintains professional affiliations and any required certifications.
  • Performs other duties as required or requested.

 

ACCOUNTANT - Colchester Center

Primary Relationships:  Administrative & Fiscal Services Manager, Executive Director, Center Manager, co-workers

 

Objective:  The accountant works with the administration to perform basic accounting functions.  The major areas are:  ledger/fund maintenance, cost analysis, fund reconciliation, posting, and fiscal control of inventory; prepares financial reports from standard operating statistics and/or financial data; investigates and resolves problems related to funds, budgets, expenditures, and/or procurement practices; provides assistance in the development, implementation, and management of fiscal systems and procedures, applying basic concepts, methods, and techniques.

  • General ledger operations
  • Reconcile and maintain balance sheet accounts
  • Assist with monthly closings and preparation of financial reports
  • Transacts wire transfers as directed by Administrative and Fiscal Services Manager
  • In conjunction with the Administrative and Fiscal Services Manager, organizes and prepares documents for annual audit
  • Reconciles monthly bank accounts
  • Assists in the preparation of the annual budget
  • Performs payroll functions
  • Manages petty cash accounts
  • Assist with the implementation and maintenance of internal financial controls and procedures
  • Manages claims adjudication process; liaison to the Third Party Administrator
