Welcome to PACE Vermont!

We are a nonprofit healthcare provider for seniors that offers Vermont’s only PACE program. PACE is the Program of All- Inclusive Care for the Elderly.

PACE is a health care program that helps older adults remain independent in their own homes. We provide direct medical services as well as home care, medications, transportation, social support, and more. PACE helps Vermonters age in place with our unique team approach to care.

We invite you to explore this website to learn about our services, get a taste of what our participants experience, and find out how to get involved as a volunteer or employee!

We also invite you to contact us to discuss PACE as a healthcare option for yourself, a friend, or a relative, or to set up a tour of one of our Health Centers.

PACE Vermont, Inc. operates two PACE centers:
• The Colchester center opened in April, 2007 and is located on the Fanny Allen Campus. This center serves all of Chittenden County, plus the towns of South Hero and Grand Isle. The Colchester center also houses the administrative offices for PACE Vermont.
• The Rutland center opened in February, 2008. It is located at The Maples Senior Living Community. This center serves all of Rutland County and the towns of Dorset, Manchester, and Rupert.
