In-Home Care Licensed Nursing Assistants (LNAs) and Personal Care Attendants (PCAs) visit participant homes to provide assistance with tasks such as personal hygiene and bathing, getting dressed, transferring, preparing meals, grocery shopping, and light housekeeping. In-home care is organized by PACE's Home Care Coordinator, who is a Registered Nurse. The Home Care Coordinator makes home visits for nursing care, and supervision and training of caregivers. PACE employs LNAs and PCAs, and also works with local providers to provide in-home care. All PACE employees receive training and must have at least one year of experience working with elders before working at PACE. The schedule and amount of in-home care is part of the participant’s Care Plan. The Health Team talks with PACE candidates and their families to determine what is medically necessary for the person to remain safely at home. The PACE philosophy is one of "doing with" participants, rather than "doing for." Participants are active in their care, and are supported and assisted to do as much for themselves as they can. Whenever possible, the Health Team helps participants become more independent by providing therapy, training, and equipment. This service is just one benefit of the PACE program. PACE participants receive all their health care from the PACE Health Team and Provider Network. Last Updated 06/22/2011 |
