How does it all Work?

     

Each participant works with the Health Team to write their own unique Care Plan.  The PACE Health Team works closely with participants and families to identify the participant's personal goals and needed services to support the participant at home. 

Care Plans are written every 6 months or whenever there is a significant change. 

All services identified in the Care Plan are paid for in full by PACE Vermont.  

To help families decide whether PACE is right for them, a Care Plan is written before enrollment, at no cost. 

PACE participants must receive all needed healthcare, including primary care and special physician services from PACE Vermont or from an entity authorized by PACE Vermont. Participants may be fully and personally liable for the cost of unauthorized or out–of–Network Services.  Please see our Provider Network for a complete list of providers currently available through PACE.
