Health Team

The Health Team includes a doctor, nurse, physical therapist, occupational therapist, social worker, nutritionist, activities director, caregivers, drivers, and the PACE participant.

The PACE participant may also invite their family members and others onto their Health Team.

Other health care providers may also be brought into the team, depending on the individual participant; for example, a speech therapist, cardiologist, or psychiatrist.  Please see our Provider Network for a complete list of providers available through PACE.
