The Intake Coordinators at PACE are available to walk you through enrollment into the PACE program.

The enrollment process includes an initial meeting with an Intake Coordinator, followed by Assessments with each of the members of the Interdisciplinary Team.  Assessments are usually spread over 3 visits to the PACE Center.

You may also need to apply for the Choices for Care program and submit a Medicaid Application.  The Intake Coordinator can assist you through this process.

Eligibility criteria:

• 55 Years of age or older
• Certified by Vermont to need nursing home level of care (the Intake Coordinator can help with this step)
• Reside in Chittenden and southern Grand Isle Counties for the Colchester program; Reside in Rutland and northern Bennington Counties for the Rutland program
• Desire to live at home and can do so safely, as determined by PACE Vermont Interdisciplinary Team
