End of Life Care

PACE supports elders wherever their care needs take them.  PACE provides additional in-home services and supports at the end of life.  PACE can also arrange for nursing home or hospital care. 

This service is just one benefit of the PACE program.  PACE participants receive all their health care from the PACE Health Team and Provider Network.
