PACE Vermont, Inc. was incorporated as a Vermont domestic non-profit corporation in December 2003. The Chittenden and Rutland long term care coalitions chose to develop PACE with a collaborative approach to governance and administration of two PACE Centers to serve frail older adults. The PACE Vermont Board of Directors includes representatives from the two Coalition’s organizations including senior housing, home care and hospice, nursing homes, hospitals, senior centers, transportation providers, mental health agencies, area agencies on aging, adult day programs, physicians and community representatives.
PACE Vermont is proud to provide the first PACE program in a rural setting. The PACE model began in San Francisco in 1973. There are now 60 PACE and pre-PACE programs across the United States. Please visit the National PACE Association for a complete timeline.
National PACE Research results show that: • PACE provides better health outcomes than other traditional care for seniors with chronic care needs • The staff serving these individuals are more satisfied working in a PACE environment • PACE is an effective use of taxpayer dollars.
