About Us
PACE Vermont, Inc. was incorporated as a Vermont domestic non-profit corporation in December 2003, and opened for business in 2007. PACE Vermont is supported by On Lok and Volunteers of America. On Lok, the pioneer of PACE, is a non-profit organization that serves seniors through a PACE program, senior center, intergenerational program, and housing. Volunteers of America is a national non-profit organization providing human services in 44 states to over 2 million at-risk and vulnerable persons per year. PACE operates under a 3-way agreement with Vermont's DAIL (Department of Aging and Independent Living) and the federal CMS (the Centers for Medicare and Medicaid Services). PACE Vermont is proud to be among the first PACE programs to operate in a rural setting. The PACE model was developed by On Lok in San Francisco in 1973. There are now 75 PACE programs in 29 states. Research about PACE: Locally, UVM’s Master of Science program found that PACE Vermont’s enrollees have fewer visits to the emergency room, fewer hospital admissions, and spend fewer days in the hospital when they do get admitted in a 2009 study comparing utilization rates pre-and post- PACE enrollment. The findings highlight a decrease in acute care utilization for falls, accidents and fractures. Research on the PACE model continues.
