Welcome to PACE Vermont!
We are a nonprofit healthcare provider for seniors that offers Vermont's only PACE program. PACE is the Program of All- Inclusive Care for the Elderly.

PACE is a health care program that helps eligible older adults remain independent in their own homes. We provide direct medical services as well as home care, medications, transportation, social support, and more. PACE helps Vermonters age in place with our unique team approach to care.
